Borden Avenue Veterans Residence Safety Evaluation


Introduction

This survey is being conducted to evaluate conditions at the Borden Avenue Veterans Residence, currently the only Veterans Affairs Grant and Per Diem (GPD) transitional housing program serving veterans in New York City. The purpose of this assessment is to gather direct feedback from veterans regarding safety, living conditions, access to services, staff support, and the overall effectiveness of the program in helping veterans transition toward stable housing and civilian reintegration.


Your feedback is important in determining whether the program is meeting the intent and standards of the U.S. Department of Veterans Affairs Grant and Per Diem Program, which is designed to provide safe, supportive, and recovery-oriented housing for veterans experiencing homelessness.


Survey results will be compiled and shared in summary form with the New York City Council Committee on Veterans, the U.S. Department of Veterans Affairs, and other oversight stakeholders to support accountability, transparency, and recommendations for improvement. Individual responses will remain confidential and no personally identifying information will be publicly disclosed.
